If your company is eligible, check with your human resources or community relations department for the proper form to request that your company match your gift. Some companies even match gifts made by retirees and/or spouses. Please send it completed and signed with your gift. We will do the rest. The impact of your gift to this organization may be doubled or possibly tripled!

The ALS Association Golden West Chapter is recognized as exempt from federal income tax under section 501 (c) (3) of the Internal Revenue Code and is registered under statutory laws throughout the U.S. Gifts to the Golden West Chapter are fully tax-deductible to the extent allowed under the Internal Revenue Code.
